TP.1000 form is missing the lower part which includes the signature and the date. This problem exists on the screen and in printing (2022)
I am unable to reproduce what you are seeing - both the signature and date show for me. Could you contact support and screen share so they can help troubleshoot what might be happening?
I contacted tech support. It’s a screen resolution issue. The resolution must be that recommended by windows.
